We now have one form that covers all these requests. Please fill out the new form and submit with appropriate documentation to your Regional Training Advisor. Regional Training Advisor's addresses are on the form. The Regional Training Advisor will make a recommendation and send to HSI Wellington office for processing.

This checklist has been designed for those providers who intend to submit their assessment material for pre-moderation to HSI.

As of 1 Jan 2009, NZQA will cease to charge for Hook-ons and therefore the Hook-on form will become null and void. However there is a still a requirement for trainees requiring a NSN to register with NZQA and as such the Hook-on form has been replaced by the Registration Form.
